Your First Visit

Please assist us by providing the following information:

• Your referral slip and x-rays from your referring dentist (if you are being referred; referral not required)
• A list of medications you are currently taking
• If you have dental insurance, please bring your insurance cards with you to the appointment
• Your patient registration and medical history forms will be texted to you at the time you make your appointment; please try to complete these before you arrive at our office for your appointment.
  

Please notify the office if you have a medical condition that may require antibiotic medication prior to your visit and examination such as artificial heart valves or a history of joint replacement surgery. 

At your initial visit, we will complete an in-depth medical and dental health history and a thorough examination including oral cancer screening, measurements of the gums, bone and loose teeth, your bite and jaw joint and other signs of periodontal disease.
